is there any background info on the Jedi who went to Kamino and placed the order for the clone army?
How did it escape the notice of the Jedi Order?
Under what circumstances was he killed?

All I remember is that he was killed by Dooku.

Since nobody seems to know who the hell he is you can now create your own wacky EU!! You could put him in situations so that he'd know everyone in the galaxy just like all other EU!!!

Sifo-Dyas was once a good friend of fellow Master Dooku (http://starwars.wikia.com/wiki/Dooku), and had the gift of precognition (http://starwars.wikia.com/wiki/Force_Vision). He predicted the coming war, and knew the Galactic Republic would soon face dark times (http://starwars.wikia.com/wiki/Clone_Wars).
Sifo-Dyas secretly commissioned a clone army (http://starwars.wikia.com/wiki/Grand_Army_of_the_Republic) on Kamino (http://starwars.wikia.com/wiki/Kamino), ostensibly to defend the Republic. Prior to this, Dooku had left the Jedi Order, and had fallen in league with Darth Sidious (http://starwars.wikia.com/wiki/Palpatine). He was told by the Sith Master (http://starwars.wikia.com/wiki/Sith_Master) about Sifo-Dyas' actions, and Sidious also revealed that they could use the clone army for their own ends. It was Sidious' true intention to use Sifo-Dyas only as a dead-end cover for Palpatine's order of a clone army.

Dooku used Sifo-Dyas for a blood transfusion for the General. Edit: Slicker allow me to addend what you said about the EU. It's not that we shouldn't have it, but not just anyone should be allowed to write whatever and leave it at that. If it's going to be done, at least it should be done by someone who has a little experience with what's what.

It's not that we shouldn't have it, but not just anyone should be allowed to write whatever and leave it at that. This comes from the same company that allowed Chewie to be killed by a rock, Palpatine to be cloned numerous times, Boba Fett escaping the Sarlaac numerous times, Dengar, Jabba having a nephew/son, Greivous created on Geonosis, Anakin having a padawan, an Old Republic Jedi turned Sith hundreds of years in the future, ect.

The whole thing about the frozen body and blood transfusion comes from Visionaries, which a lot of people tend to forget is not canon, even by EU standards.

The rest comes from Labyrinth of Evil, which is one of the bright spots in the EU. One of the really bright spots, in fact.

When I first heard him say that it was obvious in Ep 2, my jaw dropped, I was totally incensed. Granted, it wouldn't have been difficult to leap to that conclusion initially, but there's nothing OBVIOUS about it off the bat and his adding a bunch of little side anti-clues only muddled the issue horribly.
